Pathology

Syndrome of pain and stiffness, usually in neck, shoulders and hips. Associated with the HLA-DR4 and Giant Cell Arteritis .

Aetiology

The cause is unknown but there is suggestion of environmental factor possibly a virus causes immune activation in genetically predisposed patients

Signs

Symmetrical joint stiffness, low grade fever and weight loss.

Symptoms

Joint stiffness, malaise, fatigue, Difficulty completing daily life activities, Morning stiffness for more than 1 hour, Muscle stiffness after prolonged inactivity

Investigations

Bloods: FBC, ANA, RF, and CK – Usually normal
Raised ESR is the most sensitive test for PMR

Treatment

Medical: Low dose Corticosteroids 

Prognosis

With prompt diagnosis and adequate therapy, patients have an excellent prognosis. The average length of disease is 3 years
Associated with development of Giant cell Arteritis
