Pathology
Unknown pathogenesis affecting Trigeminal Nerve.
Aetiology
Idiopathic, MS, vascular compression, tumors
Signs
No clinical signs
Symptoms
Painful disabling sensory symptoms in the distribution of the trigeminal nerve.
Investigations
Imaging: MRI to assess for underlying cause
Treatment
Medical: Neuropathic Analgesia: Carbamazepine, gabapentin, and lamotrigine.
Severe cases: IV phenytoin, nerve injection with alcohol or glycerol
Surgical: Microvascular decompression, partial rhizotomy of nerve
Prognosis
Usually responds to neuropathic analgesia.
Spontaneous remission may occur
