Pathology

Arthropathy caused by calcium pyrophosphate (CPP) crystals

Aetiology

Primary or secondary hyperparathyroidism, haemochromatosis, hypothyroidism, hypophosphataemia, Wilson’s Disease.

Signs

Joint effusions

Symptoms

Monoarthritis, painful, swollen, hot, erythematous joint for 7-14 days
Most common sites are shoulders, knees and wrists

Investigations

Joint aspirate: Positively Birefringent Rhomboid-shaped crystals
Imaging: X-Rays show Chondrocalcinosis

Treatment

Conservative: Rest
Medical:  NSAIDs, intra-articular steroids

Prognosis

Destructive changes like osteoarthritis
Degenerative arthritis
