Pathology

Narrowing of tricuspid valve reducing outflow into right ventricle

Aetiology

Rheumatic fever, CHD, Carcinoid, Obstructing pacemaker lead

Signs

Raised JVP, Prominent ‘a’ waves, Mid diastolic murmur loudest in inspiration at the lower sternal edge, hepatomegaly and ascites.

Symptoms

Leg swelling, Fatigue

Investigations

ECG: P Pulmonale (Right atrial enlargement) or P Mitrale if co-existent mitral 
stenosis
Chest X-Ray: Enlarged right atrium 
Echo: Assesses for valvular and leaflet abnormalities

Treatment

Medical: Salt restriction 
Diuretics for peripheral oedema
Surgical: Balloon Dilatation, valve is rarely replaced

Prognosis

Related to underlying surgery
Surgical complications, bacterial endocarditis, prosthetic valve failure
